Alcatel/Lucent Members-Dependent Verification DUE by May 16 |

If you have not yet submitted your information, you have less than two weeks to meet this critical deadline, which, if missed, could result in loss of health care coverage for your dependents.
A package was mailed to your home the week of February 11 from Budco, the third-party organization acting on Alcatel-Lucent's behalf, requesting information for each of your covered dependents. You were asked to provide documentation confirming that your covered dependents are, in fact, eligible for coverage under the Plan rules. You were also asked to report any ineligible dependent who needs to be removed from coverage immediately. Employees will receive confirmation letters from Budco advising whether each of their dependents is eligible or incomplete after Budco has reviewed their documentation. After the conclusion of the verification period, Budco will mail disposition letters to employees whose dependents are ineligible for coverage. Please note that coverage will be dropped on July 1, 2008 for any dependent who is found to be ineligible as a result of this verification process, as well for any dependent for whom no or incomplete documentation is provided. A summary of the eligibility requirements was enclosed with the letter you receive from Budco and they were explained in detail in the Open Enrollment materials you received late last year and can also be found at Alcatel-Lucent Benefit Answers Plus.
